    Faith - What is your faith or belief? Do you consider yourself a religious person? Does religious faith or spirituality play an important part in your life? What do you feel gives life meaning?

    Influence - How does your religious faith or spirituality influence your thoughts about health? How does it affect the way you take care of yourself?

    Community - Do you consider yourself part of a spiritual or religious community or congregation? How is that community or congregation a source of support for you?

    Address - Do you have any religious or spiritual issues or concerns that you'd like me to address with you? Is there someone else you would like to speak with about these matters?
